I hope this letter finds you well. My name is Melissa Smith and I am respectfully requesting medical assistance for dental needs. The assistance is urgently needed due to my oral condition it has impacted my health extremely.
I am writing to share my situation and inquire about potential assistance or options for dental care.
About five years ago, I was in an abusive relationship, which I am thankfully no longer in. During that time, I had a couple of teeth knocked out. I did not have insurance or the financial means to replace them at the time, and over the years, the condition of my teeth has worsened, leading to severe decay and additional missing teeth.
The treatment is crucial for my health. Also it has had a great effect on my family especially My 13-year-old daughter has attempted suicide due to the fact she has been bullied and teased because of the condition of my mouth. It has played such a negative impact on the life of my family and I in so many ways (financially, mentally)
I am writing to share my story and request your support with a difficult situation I am currently facing.
I am a single mother of four girls, and an only child to a 76-year-old mother who has gone above and beyond to help support us. While I have earned an associate degree in business, I have faced severe challenges utilizing it due to dental issues. As you will see in the attached pictures, these visible challenges have deeply impacted my professional life.
In the past, I have held positions where I wore a mask during the interview and for the first couple of months on the job. Unfortunately, once I became comfortable enough to remove the mask and my teeth became visible, I have consistently been let go or denied further employment opportunities.
I am reaching out in hopes of finding guidance, assistance, or dental resources that can help me overcome these barriers so that I can successfully provide for my family and build a stable career.
My insurance only covers a small part, and I cannot pay the rest at this time due to my financial hardship and the appearance I am writing to formally request the amount of $2,500 to be sent directly to the dentist who will be performing my required dental procedure.
If you have any questions or require any paperwork or additional information from my end, please let me know. I am more than happy to provide whatever is needed to facilitate this request.
